Home therapy vs centre-based therapy: which setting fits your child?

The best setting depends on the goal, the child, the family and where the skill needs to work. Many children benefit from a coordinated combination.

Families often ask whether therapy at home is better than therapy at a centre. There is no universal answer. The right setting depends on the child’s goals, attention, comfort, family routines, available equipment and the environments in which new skills must be used.

When home-based support can help

Home sessions allow a professional to observe real routines and work with the materials, people and challenges already present. They may be useful for goals involving:

  • Dressing, feeding and self-care
  • Morning or bedtime routines
  • Family communication
  • Transitions within the home
  • Parent coaching
  • Applying a skill where it is naturally needed

Home support can reduce travel and make caregiver participation easier. However, homes also contain distractions, limited space and established patterns that may make focused assessment or practice harder.

When centre-based support can help

A centre can provide a structured environment, specialist materials and easier access to multidisciplinary observation. It may be useful when a child benefits from:

  • A predictable clinical space
  • Specific assessment materials or equipment
  • Carefully graded distractions
  • Joint review by several professionals
  • A clear separation between home routines and focused sessions

Skills demonstrated in a centre still need to transfer into daily life. A child completing a task in a quiet room does not automatically mean the skill will appear at home, school or in the community.

Ask where the outcome needs to live

Start with the functional goal. If the goal is “get dressed with less help,” home observation may reveal barriers that a clinic cannot. If the goal requires specialised assessment, the centre may be the right starting point.

Many plans use both settings over time. What matters is coordination: shared goals, communication between professionals and caregivers, and regular review of whether skills are generalising.

Questions to guide the decision

  1. What exact activity are we trying to improve?
  2. Where does the difficulty usually occur?
  3. Which setting allows the clearest assessment?
  4. How will parents and teachers be involved?
  5. How will a centre-based skill be practised in daily life?
  6. Is travel helping or exhausting the child?
  7. Can the team combine settings when appropriate?
